Additional Office Policies

We want to ensure that your time at Created to Thrive is an enjoyable one.  Wait times are minimal as we value your time.  To help us to continue to provide these services, please follow the below policies.

Respect & Dignity:  We require that our staff treat all patients and their families with the respect & dignity they deserve.  Similarly, we require that all patients and their families treat our staff with the respect & dignity they deserve.  Threatening and/or abusive language or behavior will not be tolerated.  

Any threatening and/or abusive language or behavior that results in our staff feeling unsafe and/or at risk of harm will result in termination of a patient’s appointment.  Payment for the full appointment will still be due as if the full appointment was completed.  In addition, this may result in termination of the patient-physician relationship (unless a “behavioral contract” is offered, at the physician’s discretion, and the patient and family agree to the conditions of this behavioral contract).

Reminders:  Active patients consent to receiving emails, texts, and phone call reminders for appointments unless they specifically opt-out.

Medication Refills:  Refills are provided during appointment times.  It is the patient’s responsibility to schedule the next appointment at least 1 month in advance to secure an appointment before running out of medication.  Any refill requests that are made between scheduled appointments will be provided, at the physician’s discretion, for a fee of $25 per prescription.

Forms:  Routine school/work excuses are provided upon request during your appointment time.  If time allows, brief forms (<5 min) may be completed during your appointment time without charge.  Please notify your provider if forms/letters are requested at the beginning of your appointment.  Longer forms and letters can be completed outside of your appointment time for a fee.  The fee is based upon the time involved to complete the service (at a rate of $250/hour, to be billed in 10-minute increments).  

Phone calls:  Calls lasting longer than 10 minutes will be charged based on time (at a rate of $250/hour, to be billed in 10-minute increments).  In the case of an emergency, please call 911 or go to the emergency room.    

Record Requests:  Upon written notice, records can be copied.  Please allow 1 week to have copies made.  Copies can be picked-up or mailed to the patient’s address only.  No faxes are provided.  The fee is $25 for the first 20 pages and $0.50 for each subsequent page. 

No Smoking on the premises.

Complaints:  We would appreciate notice of any concerns or complaints be sent to contact@createdtothrive.org so that we may improve any deficiencies.  Complaints about physicians, as well as other licensees and registrants of the Texas Medical Board, including physician assistants, acupuncturists, and surgical assistants may be reported for investigation at the following address:
